REGISTRATION

Registraion for the Carolina Swim Camp is a three (3) part process.

Step #1: Complete the registration form

The registration form is online only. Please click on the link below to view and complete the registration form. When completed with the form click "submit". Your form will go directly to the staff at the Carolina Swim Camp. At the end of each week, from today through the start of camp, the staff will respond by email to all parents who have registered for camp. Please be aware that the email confirmation is not immediate when you register for camp, but rather at the end of the week that you have registered.

Step #3: Complete the Medical Forms

Please download the Physican's permission and Camp Waiver forms and complete the medical information form. The physician's permission and waiver forms may be completed and returned when you arrive on the first day of camp for check-in.

Step #2: Payment

Full payment for camp is due by May 15, 2012. To reserve your spot in our camp, a non-refundable fee of $100 is required. The balance of camp will be due by May 15, 2012. Should you happen to register for camp after May 15, 2012, full payment is due at the time of registration by credit card. For the deposit or to pay in full prior to May 15, 2012 we have two options for you to choose from this year:

Option 1: You may send a personal check, cashiers check or certified check made payable to the "Carolina Swim Camp" to the following address:

**Please be sure to indicate with a note or on the check itself the name of the camper(s) you are paying for.**

Option 2: You may pay by credit card by visiting our "CAMP STORE" page. Please choose the camp session that you've registered for and the quantity of campers at each session. Add any additional items you wish to purchase and "check out."

**Please be sure that if paying by credit card, that the credit card information matches the parent's name on your campers registration form. If the name on the credit card is differerent than the parent who made the camp registration, please be sure to indicate the name on the credit card on your registration form.**
